The fireplace surround appears to have been custom made, most likely cast from concrete. The fluting detail was designed to match other fluted wood pieces in the home. It certainly cannot be made from wood as you cannot put wood right up to a fireplace opening like that.

I have no idea who could make it near Salt Lake City. This particular job was in a multi-million dollar home here in Brentwood CA. I specialize in custom fireplace screens and doors and have had them made for many fireplaces similar to this over the years. I can probably come up with a resource I can work with to have it made from a cast stone material that is much lighter weight than concrete and have it shipped to you.

Have you already built the fireplace? Is it a prefab fireplace or masonry? If so, post some pictures of the actual fireplace along with the dimensions and position of the opening and I'll be happy to poke around and see what I can come up with. Thank you so-much for the quick response and offering help. Our existing fireplace is an existing prefabricated gas fireplace made by Lenox. I will send a photo so you can see what it looks like now. I just want to get rid of the tumble marbled that we had used and make it looked similiar to the photo of the one I saw. I love the idea of the cast stone and would love any more info you can find .The insert measures 31 & 1/2inches high and 34 wide (if i measure correctly . My husband is a pro construction person so I would have him remeasure to make sure if you find any leads on making it .Thanks again and I so appreciate your help with this.!
